After asking recommenders if they can write you a favorable recommendation and getting a positive response, ask if they would indicate how long they have known you, for what time period, tasks completed, and personal qualities observed in 
their reference. An outline of tasks accomplished and your work together can be a helpful reminder for a recommender. 

A personal address to each position is preferable, but if you are applying to many positions, it may be easier to address the letter "To whom it may concern:", so you can make multiple copies to include with a cover letter or can leave with interviewers.
